This is a brand new initiative of the Eastern Metropolitan Region Palliative Care Consortium (EMRPCC). The PCCJC is aimed at providing information, mentoring and guidance to health professionals of any discipline who want to provide quality end of life and palliative care to clients/patients/residents in any setting (aged care, community, primary healthcare, hospital etc). Victoria’s end of life and palliative care framework – A guide for high-quality end of life care for all Victorians was released by the Victorian Government in July 2016 and states “All Victorians are entitled to high-quality end of life care that relieves pain, distress and suffering and provides support to their family, friends and carers. It is no longer possible to leave end of life care to palliative care providers. Current end of life and palliative care models are unsustainable and will not meet future demand without significant redesign.
The end of life and palliative care framework requires the healthcare, human services, social and community sectors to connect and work together to develop innovative new strategies to deliver care. Importantly, the framework redefines end of life care and guides providers across all sectors to take responsibility for delivering high-quality end of life care”.
The PCCJC is designed for small group sessions to enable discussion, debate and problem solving of any end of life care issues. It will improve your knowledge, assist you to gain confidence to care for people at the end of their life and give you the tools and resources needed to take back to your workplace to improve the quality of care provided.
