Specialist Palliative
Care Nurse Consultants are available in each of the acute public
hospitals within the Eastern Metropolitan
Region. These Nurse Consultants can liaise with the treating
medical or oncology team and with Palliative Care Physicians.
Inpatient Palliative Care can occur in designated units such
as Wantirna Health or may be delivered within a general ward.
Palliative Care within a hospice setting is available at Caritas Christi Hospice.
Outpatient medical and nursing palliative care consultancy plus
allied health assessment is offered at Yarra Ranges Health in
Lilydale. Palliative care (including end of life care) can also
occur within at patients own home, with support from their General
Practitioner, specialist community palliative care services
(Eastern Palliative Care) and community care providers.
The Eastern Metropolitan Region Palliative Care Consortium aims
